Accidents and Dangerous Occurrences
This section provides guidance on reporting incidents and describes NOPSA's role in investigations.
Accident/Incident Investigation Policy Framework
Following the Ministerial Council on Mineral and Petroleum Resource's acceptance of recommendations to improve performance and consistency in offshore petroleum regulation activities, the Technical Working Group - consisting of both Commonwealth and Designated State/NT authorities - prepared a discussion paper on accident/incident investigation [88KB pdf].
The paper lays out the policy framework agreed with the Designated Authorities.
Reporting Guidelines
A summary of guidelines for reporting accidents and dangerous occurrences is provided in the document: Guideline - Notification and Reporting of Accidents and Dangerous Occurrences. This includes the emergency contact telephone numbers.
Additional advice is provided on reporting incidents that occur near a facility.
Operators of offshore facilities are required to submit a monthly summary of incident information. The timing and format of this report are described in the Monthly Summary guideline. We recommend the use of this template which is provided in MS Word format.
